Home Page Name: Is the OID of the current template being defined.
Template Name: Is the name of the template that is used by this template.
Template OID: Is the OID of the template that is used by this template.
Opt: Is R, R2 or O depending upon whether the template is required, required if known, or optional within the template being defined.
Comments: Provide any additional text about template being used.
