Rationale
The removal of CRS from the Technical Framework lost level 1 conformance requirements. This CP adds them back in by reference to an HL7 balloted document (the CDA4CDT History and Physical), without the CRS specific detail. These requirements include: Names, telecommunication, and address information for various participants described in the document

Appendix ? of the CDA4CDT History and Physical Draft Standard for Trial Use.
Provide a reference to the Schematron constraints from that document.
Change the Medical Summary Section to remove references to CRS.
