White Paper: Care Management Content/Infrastructure

White Paper to review technical capabilities among standards to accommodate care management and, therefore connection of measures to guidelines and care management processes within the EHR. There are clear implications about clinical decision support within the evaluation, based on the data model. HL7 care record Care Plan will receive focus in this regard.
