1. Brief Description of the Problem:

1 or 2 paragraphs that give a rough description about why the profile is needed.

2. Key Use Cases

  • Use Case 1

There may be multiple use cases. If so, please list with a short name. For each use case, list the sequence of steps that describe how the information is created and used assuming your profile was implemented.

For each step, please be sure to include how any information created/inputted:

  • Describe what data is created (eg Vital signs, Goals of therapy, etc).
  • list the person/role (eg ER physician, Home Health Nurse, etc) that inputs the information. If the system is calling from stored information, state how that information got into the system
  • list the type of system (eg ambulatory EMR, Emergency Department Information System, Home Health, etc).

For information that is used:

  • list how the information gets to them (eg, local transaction, through a RHIO, cdrom, etc)
  • list who is using the information,
  • list how the information is used


For Example:

Use Case 1: Automated Admission History and Physical Generation

Step 1: The ER nurse records the reason for visit, time of arrival and vital signs into the EDIS (Emergency Department Information System)

Step 2: The information in step 1 is transacted to the EHR system on the floor where the Primary care physician (PCP) reviews the data, along with other labs previously transacted in to the EHR from the hospitals lab system. The PCP marks in the EHR the elements he wants carried forward to his note. The PCP then dictates the admission History and Physical including:

  • Reason for visit
  • History of present illness...

Into the dictation system.

Step 3: The transcriptionist listens to the dictation on the dictation system...

4. Further Discussion.

list any other benefits or controversies, or other details that might be important

5. Resources/People that could help the technical committee.
