IHE CDA Technical Issues

In reviewing the PCC FT document and the CDA Supplement, I ran into some technical issues/inconsistencies. I will make formal CP's for them, but this is a page for a comprehensive summary.

If it was deleted on purpose (or never existed), we should probably change the CDA supplement or put in a 6.3.3.8.1 that is "Blank on purpose".
