Usage of SNOMED CT Codes in IHE Profiles

This is to be added to the cross domain Google spread sheet listing Domain, SNOMED Code, Display Name, Profile, and Value Set. This is to allow IHE to license the codes for their ongoing usage. Chris Melo has reviewed the CPN profile. Some of the list of codes lack the information and may need further work.
