- P. 133 - I am not inclined to change the example at this point. It has been a part of the TF Final Text since last year with no complaints

- There are no page numbers in the document footers
- Sec 3.4.4 - the label for Figure 3.4-1 is on the top of one page, the figure is on the bottom of the previous page
- Cells in tables between lines 3435 and 3470 (sec 3.16.4.1.5 thru 3.16.4.2.4) are filled in with grey. They are not that way in the 2009-07.27 version of the document

- This is purely cosmetic, but in the TOC, the names of the appendixes don’t line up properly. It’s as though some use spaces and some used tab between the Appendix letter and the name of the appendix.
- In Section M.1, the Figure is labeled “Figure Title: MISSING – please insert”. The original had no figure label at all. I think we could label it “Figure M.2-1: Patient Demographics Supplier in a Multi-domain Environment”
- Table V.2.4-1 on pg 71 goes off the right side of the page.
