Action Items:

CP-0053 - Tone & Mike to complete CP, Tone to make change to TF
CP-0036 - Tone to rewrite and write new for APR formatCode changes
CP-0041 - since this will now be 4 content profiles need to address this with the formatCodes in cp-0036 as well

reviewed and made some modifications, including (but not necessarily limited to):
- added references to IHE Cardiology
- filing in some examples
- added content provided by PCD
- generalized trace lead labels to trace identifiers

further steps:
- Discuss existing work going on with respect to communication of sample data in PCD, IEEE and elsewhere.
