Pregnancy history is in the history and physical section, ....and a long list of other things in Keith's notes that he read to the team.  Keith owns this CP and will write it up and submit it.

    • Have added the open issue needing resolution through discussion with ITI.
    • Have added the header "order fulfillment module".
