Discussion Summary

The committee reviewed the DIS work item proposal and discussed the proposed scope, transaction model, base standards, policy handling, and relationship to other standards work. The committee identified several open issues to be addressed during continued work item development.

DAPL Alignment / Overlap

• Alain responded that DAPL may not align directly with the DIS scope, policy assumptions, and transaction model.
• No final conclusion was reached.

Base Standards

• FHIR PlanDefinition was discussed as a possible Resource to profile.
• The proposal should clarify how it will handle FHIR version differences, including FHIR R4 and FHIR R5.
• The approach should version-agnostic given uncertainties in EHDS

Workload Assessment

• Workload assessment was not discussed.
• The proposal identifies as medium effort.
• Technical Committee to review and confirm workload estimate.

Motion

Motion: Accept the Phase 1 DIS work item for the current quarter, noting the unresolved issues identified during discussion.

Issues noted with the motion:

• Missing transaction or interaction pattern to retrieve task results/output after task submission.
• Policy development is currently considered out of scope, but remains an open item for clarification.
• Base standards remain to be finalized, including use of FHIR PlanDefinition and handling of FHIR R4/R5 differences.

Key Discussion Points

DICOM

• DIS should leverage existing DICOM de-identification standards and profiles.
• Transactions should reference DICOM Attribute Confidentiality Profiles and image retrieval locations.
• The proposal should reuse DICOM PS3.15 confidentiality profiles and associated de-identification evidence mechanisms where possible.

FHIR

• There is currently no IHE profile for FHIR de-identification.
• The group discussed examples from the IPS de-identification handbook.
• PCC could potentially develop FHIR de-identification profiles.
• Related work includes HL7 DARTS and DAPL, which define FHIR-based privacy transformation services and privacy-safe data profiles.

Pseudonymization

• Pseudonymization was identified as a gap area.
• No existing profiles were identified to support pseudonymization.
• Pseudonymization remains outside the initial DIS Phase 1 scope.

Technical Committee Feedback

• The Technical Committee indicated that DIS should profile an existing standard.
• The committee does not want to proceed with a profile based solely on generic REST/HTTP interactions.

Next Steps

• Continue discussion at the upcoming face-to-face meeting. Face-to-Face Agenda: ITI Face-to-Face Agenda
• Invite participation from IHE Radiology (RAD).
• Further evaluate DICOM and FHIR profiling approaches for Phase 1.
