- The example has <entryRelationship type='REFR' ... > when the attribute name should be "typeCode".

Fixed closing tag
- In the example, the closing tag to the top level entryRelationship is
<entryRelationship> when it should be </entryRelationship>.

Placed templateId on supply act to remain concistent with CCD
- In one place, I believe the example is correct while the lower passage is incorrect. The requirement for <templateId
root='1.3.6.1.4.1.19376.1.5.3.1.4.7.3'/> being present is listed twice. One place states that it shall appear under <entryRelationship>. Later it states that it must also appear under <supply>. I'm guessing that that example is correct when it lists that templateId only under <entryRelationship>.
If, however, that template ID is indeed supposed to be attached to both the <entryRelationship> and the <supply>, then it is the example that is missing a template ID.
