Corrected XPath
Minor issue: The XPath "patient/genderCode" in the "HL7 Clincial Document Architecture, Care Record Summary or Continuity of Care Document" column of the first table appears to be incorrect. "genderCode" isn't a valid child of patient in CDA R2, so I'm assuming "administrativeGenderCode" is what was intended.

Minor issue: The XPath "patient/dateOfBirth" in the "HL7 Clincial Document Architecture, Care Record Summary or Continuity of Care Document" column of the first table appears to be incorrect. "dateOfBirth" isn't a valid child of patient in CDA R2, so I'm assuming "birthTime" is what was intended.

Minor issue: The XPath "patient/ethnicityCode" in the "HL7 Clincial Document Architecture, Care Record Summary or Continuity of Care Document" column of the first table appears to be incorrect.
