The sentence: "While mappings have been provided to various standards, this content module conforms to the ASTM/HL7 Continuity of Care Document as well as this guide" combined with Section 1.4 of CCD means that a XPHR Extract shall have the Template ID denoting CCD conformance (2.16.840.1.113883.10.20.1) in the header.  However, the schematron and example omit this template ID.
