Changed name to Assessment and Plan as was intended for this section

I'm not sure if this is an issue or not. This section is labeled "Care Plan". However the description calls it an "Assessment and Plan Section". Assessment and Plan Section is also what the link from ED Physician Note calls it. Additionally, the original IHE PCC-TF 3.0 PDF files refers to this as Assessment and Plan. In the example XML, the section is referred to as Assessment and Plan, but the schematron calls it a Care Plan Section.

(1.3.6.1.4.1.19376.1.5.3.1.3.31 is also labeled "Care Plan").
