Rationale

Connactathon format codes have been wrong for two years, we need to document the correct values clearly in the TF in a way that this information cannot be mistaken

Content

To each content definition, add the code value, and display name to use the value of the formatCode metadata.

Create a table in the "Namespaces and Vocabularies" section of volume II that lists the format code, the appropriate display name for it, the mediaType that shall be used on documents using that format code, and a brief description of it:
