4 Vent Mode Taxonomy
- Furniss
Status/Discussion:
  • Chip Furniss presented an updated Breath Mode Taxonomy model
  • Operational Mode vs. BtB Breath Type annotation - Is this breath mode prescriptive as an operational mode setting or descriptive as an annotation indicating the last type of breath that was delivered. It was noted that both are needed - as separate parameters.
  • Should the operational mode setting simply be a "text label" as would appear on the front of the vent? Though this would work and function as a least common denomenator, it provides little support for applications that will use this information programmatically.
  • Therapy Objective Axis - In order to provide additional organization to the X axis, a set of Therapy Objectives was added to group various modes (namely, Minute volume / Work of Breathing / Patient-Vent Sync / Oxygenation Improvement / Lung Protection / Other Operational Therapies.
  • Discussion included other modes such as Cardinal's "CLIO2" (closed-loop Inspired O2), how high-frequency ventilation would be addressed, etc.
  • How should the problem space be modeled? Instead of bundeling everything into a single breath mode enumeration, separate parameters should be used for General Operational Mode Setting, Backup Mode Setting, Delivered Breath Type Annotation, etc.

Decisions/Issues:

  • What happens if multiple "therapy objectives" are active at the same time? Are there primary, secondary, tertiary, etc. therapies? Aren't all lung modes concened with "oxygenation" and "lung protection"?
  • Does the vent really have any concept of how it is being used from a clinical perspective, or should it simply reflect how it is configured and the types of breaths that were delivered (settings & monitors)?
  • At some point, the vent mode wiki page needs to be updated per this and other discussions (e.g., Dieter's comments) and especially definitions added for the Breath Control Parameters.
  • Todd mentioned that the group needs to make sure the discussion is rooted in the use cases that are within the scope of the IHE PCD. This is especially a problem when discussing clinical intent vs. device configuration and operation.
  • Understanding how clinicians would use this information should provide insight into how best to organize the concepts.

Action(s):

  • (Chip) Will publish an updated model to the group by 2008.06.22
  • (Todd) Provide an overview w/ use cases of the RTM Vent work to ensure that the discussions are placed in context of the real-world problems within the scope of the IHE PCD.
  • (Group) Evaluate the need and other options for organizing the X axis (vs. using Therapy Objectives)and whether this taxonomy applies to known breath mode settings.

Status/Discussion: Though scheduling of these bi-weekly WebEx sessions was initially coordinated with other related meetings, one of the groups changed their time to directly conflict with the Vent TG's schedule. Discussion indicated that the earliest for U.S. West Coast participants would be 09:00 Pacific, but the day was not as important.

Decisions/Issues: Issue a query to the group regarding an alternative scheduling for these meetings, suggesting Wednesday June 25th @ 09:00 as a starting date.

Action(s): (Todd) will issue the rescheduling request to the Vent TG.
