4:00 – 4:10 Chronic Care Workflow

• Report by Laura Heermann Langford
  • Small group met yesterday to review the current proposal. Determined the proposal has three parts to it (1. Care Planning, 2. Referral for care. 3. Encounter Summary/Note for referral visit). Concerns were expressed regarding the size of the proposed scope. Team advised to break each part into its own proposal and prioritize them. Small group will reconvene next week to address the reorganization of the scope and proposal(s). Goal is to approve proposed work in call next week.

4:10 - 4:20 Nursing e-Summary

  • Nursing Subcommittee leadership continues to work on organization and planning of the project prior to convening the larger team in January.

4:20 - 4:30 Newborn Discharge Summary

Working closely with Tone to learn the IHE process, requirements and how to focus project. Will build on model from the ante-partum summary, LOINC coding, display formatting examples etc. Currently collecting large number of paper form examples, with goal being to create a more tangible and useful display.

• Question – Is there a clear template for writing the appropriate documents that need to be created for February. Answer – Yes, Tone will send it to Alan. Until then team recommended using the Ante partum summary document as an example.

• Question – how are the information flows laid out? There will be a difference from some of the other documents. The Newborn discharge summary uses a subset of fields already contained in other profiles. Team advised to diagram what makes sense for this topic to be discussed by the larger team. The larger team will then be more effective in guiding project team to existing documents etc.

• Team has goal to have something by next week for more review and discussion.

4:30 - 4:40 Postpartum Visit Summary

• Started on the Volume 1. Earlier issues regarding scope and direction have been addressed by Tone and Mike. Current Volume 1 progress includes creation of 2 use cases and workflows for each use case drafted. Also, currently working towards postpartum visit summary as a CDA document. Goal to have more to share and discuss with team next week. This project has relatively small amount of new pieces, largely reusing existing work.
