A.) Profiles to Consider for FT

1.) List of supplements for TI that will be discussed today:

  3. Cross-Enterprise Document Reliable Interchange (XDR) - Revised 2008-10-10
  4. Cross-Enterprise Document Sharing - b (XDS.b) - Revised 2008-10-10
  5. Document-based Referral Request (DRR) - Published 2008-08-22
  6. Document Digital Signature (DSG) - Revised 2008-10-10
  7. Notification of Document Availability (NAV) - Revised 2008-10-10
  8. Patient Identifier Cross-Reference (PIX) and Patient Demographic Query (PDQ) HL7 v3 - Revised 2008-11-10
  9. Pediatric Demographics Option for PIX & PDQ - Revised 2008-11-10

2.) Short list of TI Supplements that do not qualify for Final Text:

  1. XCA is not technically stable.
  2. RFD is not technically stable.
  3. ERR has not been tested.
  4. SVS little testing has been done; one repository. Required for FT- Minimum two implementations. Therefore, disqualified not enough testing.
  5. Async Option not enough testing.
  6. DSG does not meet requirements. Only tested in one region. Requirement for FT- Minimum two implementations. Therefore, disqualified not enough testing.
  7. NAV does not have enough testing.
  8. PDO option does not have enough testing.

3.) Remaining profiles that need additional discussion based on the consensus checklist for proposed supplements.

  1. XDSB,
  2. PDQ v.3
  3. PIX PDQ
  4. XDR

Please refer to the list of questions below as a final consensus is made for each TI supplements.

  1. Has the profile been through a Connectathon in at least two regions?
  2. Has the profile been successfully tested with all actors at least at one Connectathon?
  3. Have different implementations of each actor in the profile been tested?
  4. Have all the options been tested successfully at at least one Connectathon?
  5. Are there IHE-provided software testing tools to address all aspects of the profile?
  6. Have the standards underlying the profile been implemented? In similar use cases? In healthcare? In general IT?
  7. Do you have concrete reason to believe that this works robustly in the Real World/ Are any products available for purchase that implement the profile?
  8. Have all issues that may have been raised about the profile been resolved?


a XDS.b

  • Meets all requirements on consensus page except number 8. CPs have been raised regarding meta data clean-up. No open issues with meta data clean-up at this time.
  • Reviewed the consensus check list and received a resounding yes on all items. Tested thoroughly. Used today in real world products. Resolved all major technical issues.
  • <DECISION> Approved XDS.B profile for final text with one condition:
    • 1.) There are no major issues from European Connectathon.
  • <ACTION> Tech Cmte will make Final decision in May 2009 after the European Connectathon.

b PDQ v.3

  • Meets concensus requirements numbers 1-4, 6 and 7.
  • IHE does not provide testing tools for this profile (5).
  • The fuzz search issue has not been resolved (8).
  • <DECISION> Agree to take PDQ v.3 profile to final text with three conditions:
    • 1.) Commitment to build/provide the test tools for the 2010 testing season.
    • 2.) Resolution of all technical issues and outstanding Cps and any issues that may come forth at the European Connectathon.
    • 3.) Good implementation at the European Connectathon.
  • NOTE: Sean from Axolotl is interested in writing tools for this profile.


a PIX PDQ

  • Meets concensus requirements numbers 1-4, 6 and 7.
  • IHE does not provide testing tools for this profile (5).

  • <DECISION> Agree to take PIX PDQ profile to final text with three conditions:

  • No product marketing that supports the XDR profile.
  • <DECISION> Do not take to Final Text at this time.

  • <ACTION> ITI Planning co-chairs will write an announcement (via email) to IHE News and XDS Implementors list serves that will state IHE plans to retire XDSa because IHE is not aware of vendors that depend on this profile in the field. Please send any concerns.
  • <ACTION> Review feedback from announcement at May F2F meeting and make final decision.

Review of the ITI_Editors_Guidelines
Discussed suggestion to move May meeting one day early to start on Sunday, May 3 rather than current plan to start Monday, May 4. Conclusion delayed to later in the week.

Access Control white paper

Presentation of a tentative walk through of the white paper.
Discussion about the detailed scope of the white paper. The committee agrees on the following points:

  • patient and provider safety should be added in the rationale for security part;
  • obligations (ex. documents which can't be printed or stored) and promises (ex. users committing themselves not to print the documents) should be taken into consideration in the white paper as security measures in addition to the technical measures described in the paper;
  • the number of pages should be limited to a maximum of 60 so as to still be readable for general audience;
  • chapter 7 should point out alternatives as to where IHE could go next in the area of access control without describing them into details.


Publish / Subscribe

Presentation of the outline of the pub/sub profile

Discussion of how to bind abstract publish/subscribe model to a explicit use case based profile.

  • Each use case as an option within the pub/sub profile
  • Each use case as a conent profile that binds to pub/sub as content creator and consumer (XDS model)
  • Abstract profile which requires a binding to a concrete profile in order to be implementable
  • Put common transaction information in an TF appendix and reference from concrete profiles. (Web Services Appendix model). This is ammended later to put the common material in TF-2 Chapter 4.


Multi-patients query

Presentation of the current draft.
Discussion about the scope of the profile. The committee agrees that:

  • combination with XCA is out of scope of the MPQ profile, however, the cross-community use case should be kept in mind so as not to develop a profile needing dramatic changes for extension to a cross-community environement;
  • use of BPPC should not be mandated but it should definitely be enable in order to check for consent (need of several linked multi-patients queries and/or queries).


SOA white paper

Reviewed ITI_SOA_White_Paper_Outline_ak_012109.doc.
Discussion of goal of white paper, outline and intended audience. Generated a list of concepts in SOA and concepts in IHE and the relationship between them, where it is appropriate.

  • Agreed to put common information in TF-2 Chapter 4 and use case based profiles would refer to the common parts.
  • Discussed number of supplements that should be produced, agreed to produce one supplement for now. Re-assess if other domains will be referencing the section in Chapter 4 this year.
  • Agreed that XDS publish/subscribe would be named Document Subscription (DSUB).
  • Discussed whether to include a publish transaction.

Technical committee capacity

Review of timeline and capacity for the ITI technical committee.
The committee acknowledges that this year tight schedule will be an issue to include CPs filed after the European Connectathon in april in TF vol 6. An effort will be done to clear outstanding CPs before the American connectathon in order to be able to devote time for quick resolution of new CPs.

Considering the tight schedule and to further increase efficiency of committee work, the committee agrees to the following:

  • respect of the input and output of each face to face meeting;
  • for each work item, identification of an editorial team in charge of making sure that inputs are ready for each face to face meetings through reviews and comments;
  • for each new editor (i.e. with no experience in writing a profile or a white paper), identification of a mentor in charge of helping with editorial task (template, completeness of text, publication processes...).


As a reminder, input, goal and output of each face to face meeting are listed below:

First face to face meeting of the cycle (November 18th-19th for 2008-2009 cycle)

Input:
  • Detail proposal for each proposed new work items with a list of alternatives for the standards that can be used.
Goal:
  • Based on an analysis of the detailed proposals, to define recommandations for the coming cycle in terms of new work items.
Output:
  • Written recommandation to the planning committee.


Second face to face meeting of the cycle (January 26th-29th for 2008-2009 cycle)

Input:
  • For each work item, a document started with the proper IHE template properly applied;
  • For each supplement, a rough cut of volume 1;
  • For each supplement, a list of alternatives for standards to be used for the profile with strengthes and weaknesses of each alternatives.
Goal:

  • For each supplement, review of volume 2 and preparation for public comment publication of all work items.

  • For each work item, an approved document to be released for public comment publication possibly pending minor updates to be completed before the release date.

  • For each work item, a complete document updated with resolution from non-controversial comments (done through call(s) with the editorial team);

ITI Publish / Subscribe Infrastructure

Discussion about the scope of the work and the two part of this work:

  • specification of subscription, publication and notification messages;
  • content of the various messages.

It has been decided to divide the work item into a base profile specifying content neutral messages and a content profile describing the contents of the messages for the XDS.b publish / subscribe use case.
A possible work item for a future cycle would be a white paper discussing other use cases than XDS.b and guidelines to explain how the base profile could be used to satisfy those use cases.

Cross-Community Patient Identification (XCPI)

Discussion about the scope of the proposed profile and extension to include the case when a national patient identifier is available but there is a need to efficiently identify where the records for a particular patient are located.

Discussion about the scope of the work item, the transactions involved and the appropriateness of defining the content of a policy. As no agreement was reached, decision and assessment has been deferred to wednesday morning.

Federated Service White Pages (FSWP) / Organization Registry (FOR)

Following the ITI planning committee face to face meeting, those profiles have been merged into a single profile renamed Federated Directory Service (FDS).

Though directory services already are used on the field (based on various standards), it was reported that the "cross-community" functionality (a gateway forwarding a query it received to other gateways) is a new feature that hasn't been defined through any standard yet.

Discussion about the scope especially stressing that:

  • pseudonymisation and de-identification may cover not only patients but also providers or even medical processes;
  • pseudonymisation and de-identification may be needed not only for privacy reasons but also for medical reasons.

Expanded SOA white paper

Discussion about the scope of the white paper and the fact that defining architecture isn't in the scope of IHE. The committee stressed that, at first glance, IHE profiles and SOA may appear contradictory and that one of the main goal of the white paper should be to explain how IHE profiles might be used in a SOA environment. An update to the proposal presented as an outline for the white paper and including those considerations was asked for wednesday morning. Consequently the white paper assessment has been deferred to wednesday morning.

Cross-Enterprise Privacy Policy (XPP)

Consequent to the tuesday discussion, the authors of the proposal have proposed to rescope it as an Access control white paper proposal.

The committee agreed on the new scope as a good forst step in resolving the complicated technical issues on a topic that is often identified as a gap in IHE.

Recommendations for new work moving forward

After discussion about the amount of work the ITI technical committee will be able to handle during the coming cycle, it had been decided to accept 5 work items (i.e. 2 to 4 profiles and 1 to 3 white papers). Two alternatives have been considered to reach a decision on which work items to recommand:

  • re-vote on each proposals to reach a new priorization and select the 5 top work items;
  • identify groupings of work items and vote on the various groupings.

After a vote, the second alternative (vote on groupings) has been selected by 8 votes versus 5 votes for the first alternative (re-vote on each proposals).

Identification of groupings has been made through proposals by committee members. All suggested groupings were accepted. The proposed groupings are:

The FDS proposal has consistently been proposed to be deferred due to its size and relatively low level of priority compared with other Medium of Large work items in the ordered list handed out to the technical committee by the planning committee. It was felt that once four Medium or Large work items from higher priority were selected, only a Small work item could be accepted.

The votes were cast as followed:

  • Option B1: 5 votes
  • Option B2: 3 votes
  • Option C: 5 votes
  • No abstain


As Option B1 and Option C had collected the same number of votes, it was decided to re-vote just between those two options.

During discussion prior to the vote, the main arguments presented in favor of Option C were:

  • Pseudonymisation is not a core knowledge base of the ITI Technical Committee.
  • Access Control is a core knowledge base of the ITI Technical Committee and there is an external concern that IHE does not address security adequately. Some committee members felt that we must address that concern before it is too late.
  • Some members believe that some work on Pseudonymisation can move forward without the direct involvement of the ITI Technical Committee. In July the ITI Technical Committee will consider work on this White Paper as a late season activity, similar to the 2008 work on Metadata Versioning White Paper and the update to the Cross-Community Information Exchange White Paper.


The resulting recommandation will be communicated by the co-chairs to the planning committee by December 10th and a final decision will be taken during the joint meeting between the technical and the planning committee on December 17th.

  • Cookbook on security section of IHE profiles - the co-chairs will request a volunteer from another committee to be an example of how to apply this work.
  • Template for XDS Affinity Domain Deployment Planning - still fixing several problems with current version. Additional updates are being requested, we expect a new version early next year.
  • Metadata Versioning - for this season we will keep this as a white paper and revise as appropriate based on feedback. Except to move this to an official profile next year.
