IT Infrastructure
The IT Infrastructure Domain supplies infrastructure for sharing healthcare information. An infrastructure interoperability component represents a common IT function that is used as a building block for a variety of use cases... a necessary ingredient, but rarely visible to the end user!! These components may be embedded in an application, but are often deployed as a shared resource within a RHIO or Health Information Exchange.
This domain began in 2003, is sponsored by Health Information Management Systems Society (HIMSS). The ITI domain Profiles fully described in the IT Infrastructure Technical Framework.

Current Domain Activities
2009-10 Profile Development
- Enhanced SVS (ESVS) Healthcare interoperability is enabled by the combination of syntactic and semantic consistency. IHE began addressing support for creating semantic consistency through the IHE Shared Value Set supplement profile. In taking a Registry/Repository approach to sharing vocabulary, we then have three constructs of vocabulary: Vocabulary, Metadata Vocabulary, and Vocabulary Metadata.
- Healthcare Provider Directory (HPD) This proposal recommends development of a unified approach for electronic lookup query where, upon initiated with a unique identifier or key demographic elements, an authorized user may obtain content of a unique provider entry with high reliability.
- System Directory for Document Sharing (SDDS) Currently, the XDS.b and XCA profiles do not specify how the participants in the document exchange know about the existence and web services end points of the document repositories, the document registry, and the various gateways. An XDS Document Source needs to know the web services end points corresponding to all possible Document Repository Unique IDs that are available in the XDS affinity domain. Similarly, an Initiating Gateway needs to know existence and addresses of the appropriate Responding Gateways. This proposed supplement will enhance the XCA and XDS.b profiles with options to satisfy these needs.
- XDS Metadata Update Service The existing Metadata Versioning white paper has been in circulation since 2008-10-10 and has been implemented by a half dozen vendors. This proposal will turn this white paper into a profile.
- Dynamic/Deferred Document Sharing (D3S) This proposal requests a solution for sharing health information across and within communities, or Health Information Exchanges (HIEs), that goes beyond the capabilities currently available in the Cross-Community Access (XCA) and XDS.b profiles. XCA and XDS.b address the sharing of documents created as a consequence of healthcare activity: persistent documents attested to by a clinical provider or healthcare professional. This proposal extends that work to allow for sharing of health information content that is compiled dynamically in response to a request: machine generated content built specifically because of a request for healthcare data.
- XUA Extension for Access Control Access control information is exchanged for XUA, but has very limited mandatory specification of attributes in ITI-40.
XSPA specifies a fuller vocabulary of attributes needed for access control decisions. This should be reviewed and adjusted as needed for international use.
