Corrected XPath

Minor issue: The XPath "patient/genderCode" in the "HL7 Clincial Document Architecture, Care Record Summary or Continuity of Care Document" column of the first table appears to be incorrect. "genderCode" isn't a valid child of patient in CDA R2, so I'm assuming "administrativeGenderCode" is what was intended.

"dateOfBirth" isn't a valid child of patient in CDA R2, so I'm assuming "birthTime" is what was intended.

This text needs to be fixed-->> The sentence: "While mappings have been provided to various standards, this content module conforms to the ASTM/HL7 Continuity of Care Document as well as this guide" combined with Section 1.4 of CCD means that a XPHR Extract shall have the Template ID denoting CCD conformance (2.16.840.1.113883.10.20.1) in the header. However, the schematron and example omit this template ID. <<--
